DIAGNOSTIC MANAGEMENT OF SMOKING IMPACT ON SUBGINGIVAL MARKERS IN PERIODONTITIS PATIENTS. REVIEW

Abstract

The health management of periodontal patients is extremely complex and requires cerefull consideration. Smoking is a critical risk factor for the development and progression of periodontitis. Tobacco impacts the pathogenesis of periodontitis by altering the host’s immune response to periodontal pathobionts. However, evidence of the effect of smoking on the subgingival proteome and microbiome is scarce. Analysis of the overall proteome showed that its distributionwas not significantly different between smokers, ex-smokers, and non-smokers. Smoking can increase the expression of several keratins and proteins related to hyperkeratinization of the epithelium. Smoking may induce a lower expression of proteins related  to adaptive immunity, such as immunoglobulins. This immunosuppressive effect may persist in ex-smokers.The smoking status does not significantly modify the overall GCF periodontal proteome  in subjects with periodontitis. Although smoking does not significantly modify the overall GCF proteome associated with periodontitis, it alters the expression of several proteins compared to never-smokers and ex-smokers. The knowledge and attitudes towards healthy life is not high correlated with the medical profile, because life style depends also on the family behavior, friends’ influences and educational role of the universities.

If so far, medicine universities curricula did not mandatory provide education in this domain, we hope our efforts will be useful to future generations of doctors to benefit basic knowledge in tobacco and nicotine dependence management.
